Walking With You Until You're Through
Contact Us: (218) 820-7671
Starting therapy is a BIG step. There are lots of reasons it can be challenging- from making a phone call, returning with new or evolved struggles, feelings of embarrassment, shame, sadness, pain or even hope.
We are here to walk with you on this journey. No judgement.
You can contact our admin team numerous ways - phone call, email, or even through our website!
If you reach out via email or through our website, please keep in mind that our Admin Team will need to talk to you over the phone to discuss details and how we can help you specifically.
When talking with our admin team, they will ask demographic questions, health insurance information, previous or current coordinating services, legal involvement, and the reason for seeking services/symptoms you are having.
Our admin team will work to schedule an appointment with a provider of your choice or with one that could be a good fit based on what you have shared. New clients are encouraged to read the provider bios on our website to find the best fit for them.
Our admin team will also give directions on completing digital and secure intake paperwork.
Answering the intake questions are vital to your first appointment. The more information you provide, the less questions our providers have to ask at our first session. We require digital intake forms to be completed by 8:30am on the day of your first session unless arranged otherwise.
Before you complete the digital intake forms:
If you find that a digital format is not working for you, we have options!
Why is there so many questions?
These questions help complete a Diagnostic Assessment (DA) or Integrative Assessment.
Why do I need an assessment?
There are a few reasons:
1. Health Insurance companies require a diagnosis to bill for services
2. Assessments give providers a better picture of who you are, what you have experienced in the past, what you are experiencing now, and how to help you move forward.
If you have other services that directly impact your mental health, or have in the past, we may want to coordinate care.
These are a few professionals we often work with:
If you have had a Diagnostic Assessment (DA) or Psychological Evaluation completed within the last 2 years, we would like to have a copy prior to your first appointment.
How do I get my records?*
You don't need to dig around for physical copies. Instead, you can sign a Release of Information (ROI) form with us and our admin team can request a copy of the records/assessment. *If we are needing records for a minor, a legal guardian must sign the ROI.
I need to sign a Release of Information (ROI), can I do that digitally?*
Absolutely! Our admin team can send you a ROI via email - we use a secure program called SignNow (also known as eSign).
How to sign a digital release using SignNow:
1. Verify our admin team has all the details - the reason the for ROI, who it needs to go to, what needs to happen once we have the signed ROI, etc.
2. Check your email - the subject will say Signature Request from: esign@acquirementalhealth.com If you don't see it, there is a chance it went into your Junk/Spam folder.
3. When you open this email, click on the button that says View Document
4. A screen will pop up, you do not need to download their app - scroll down to see the option to sign with mobile browser or sign in desktop view.
5. The ROI form will open. We do our best to fill in the details prior to your signature. The minimum information we need is the Client's First & Last Name, Date of Birth, Signature by Client/Legal Guardian, and Date of Signature. Press Finish when you are all done. Once you sign a digital ROI, you might receive a copy in your email for your personal records.
Our admin team is automatically notified and will handle the next steps.
*Not all agencies will accept a digital signature.

coming soon!
A key part of your therapy journey is meeting with a provider that you can trust and be open with. You deserve to feel safe and comfortable when sharing difficult things.
If you feel like something isn't working, try addressing it with your provider directly. Or you can talk to our admin team about finding a different provider.
I am not sure this is a good fit, but I don't want to hurt their feelings!
It is okay - it happens often. Good providers want what is in your best interest and if that is with another provider, that has a different approach, they will respect your decision.
Just like in real life, we don't always get along with everyone.
Frequency
After your first session, your provider might suggest how often you should meet. Often, when meeting with a new provider, they like to meet weekly to help establish a routine and build a relationship.*
*Our admin team does their best to schedule new clients weekly when it is available.
Goals for Therapy
In therapy, having goals and check-ins with progress is an effective way to make strides in your journey. Not only will you work with your provider to establish goals and the steps to reach them, but your provider will help you develop tools to reach your goals. It is your commitment as a client to use the tools and skills to make changes outside of therapy.
