FAQ: Health Insurance & Billing

Please reach us at contact@acquirementalhealth.com if you cannot find an answer to your question.

Acquire is contracted and credentialed with most major insurance companies - including: Aetna, Blue Cross Blue Shield, Cigna, Health Partners, Humana, Medica, MA (state of MN), Optum, Tricare or TriWest, UCare, & United Health Care. 


Curious how much your Health Insurance will cover? 

Each health insurance plan is unique. We won't know how much services cost, after insurance, until it has processed and been returned. 


Wanting to know the cost sooner than a statement? 

Call the number on the back of your health insurance card and ask about coverage for "behavioral health" services. 


Please keep in mind that accepted insurance plans vary by location and provider. 


I received a statement and I want to pay it, but I don't know how.

Options Include: 

1. Call our office Monday - Friday to speak with our Admin team about making a payment on your account. (218)820-7671 *extension 2

2. Scan the QR code on the statement with your smartphone camera to access an online payment portal. 

3. Drop by at our Brainerd office Monday - Friday to pay in-person. 

4. Mail a check to our main office. Addressed to Acquire, 4682 Wilderness Court #103, Brainerd, MN 56401


I received a statement and I want to pay it, but I don't have that lump sum of money right now.

Our Admin team can help you set up a monthly (or more frequent) payment plan. We have the ability to add a card on file for future payments and can set the amount to be what works best for you. 

Please keep in mind that balances over $300, with no payment plan, may result in pausing services until the balance is reduced.  


Employee Assistance Program, or EAP, is offered by employers and is considered  part of an employees' benefits. The employee, or immediate family member, is  authorized to have a select number of covered sessions by the EAP company. It is recommended to reach out to your HR department and the EAP company directly prior to scheduling. 


At each Acquire location, there are select providers that are contracted with Optum EAP, MN Fire+Optum EAP, and NuVantage. 


Can I continue services after I have used all of my EAP Sessions? 

Yes! We can continue to schedule with the same provider under your health insurance if we are credentialed.

If you do not want to use your health insurance, or do not plan to reach your deductible, we offer Self-Pay or Private Pay rates. 


Self-Pay or Private Pay is considered a "cash discount" and must be paid prior to your scheduled appointment. Please call our office (218) 820-7671 for our current rates. 


We highly recommend utilizing your Health Insurance to help you reach your annual deductible.

They have MNsure Navigators available to help assist with the application process along with finding the best plan for you.

Our providers work with individuals, couples, and families on with a wide range of issues - including communication, anxiety, depression, ADHD, mood disorders, communication, grief and loss, trauma, and more.


Before your session begins, our Admin team will send you Intake Forms. By completing these forms, you are participating in your first therapy session. These forms contain questions, history, screening tools and more. Having these forms complete will help reduce the number of questions at our first session. 




A typical counseling session at Acquire Mental Health Clinic lasts for 50 minutes, although some sessions may be slightly longer or shorter depending on your needs and preferences.


We understand that the Intake Forms that we require can be overwhelming.

These forms contain questions, history, screening tools and much more. 


The Intake Forms construct your first therapy session; Intake Forms contain questions, history, screening tools, and more. Having these forms completed will help reduce the number of questions at your first session. These forms are vital for completing a thorough Diagnostic Assessment (DA) or Integrative Assessment. 

Most importantly, these forms help the provider understand where you are at, where you want to be, and how they can meet you there. 


Before you complete the digital intake forms: 

  • Have your Health Insurance card (you have the option to take a photo) 
  • Login with the client's date of birth 
  • Clients typically take 20+ minutes to complete the forms
  • Complete the forms start-to-finish/all at 1 time to reduce information loss. Sometimes it doesn't save and that can be very frustrating. 


If you find that a digital format is not working for you, we have options! 

  • Email an unfillable PDF version of the Intake Forms for you to print at home & bring in at your scheduled appointment time. 
  • Contact our main office to set up a time to complete Intake Forms in-person. If you are needing additional help, you are welcome to bring someone with you or ask our Admin team.

A Diagnostic Assessment or DA is an evaluation completed by a mental health therapist. It focuses on the past and present needs of a client, along with struggles and measurable screening tools.


An Integrative Assessment is a mental health Diagnostic Assessment with a Substance Use component. This assessment is comparable to the (now retired) Rule 25 Assessment that is commonly requested by counties, courts, attorneys, and probation agents. 


An Integrative Assessment is similar to a Comprehensive Assessment, but can be billed to your Health Insurance or as Self-Pay.
